A record of health care services provided to you is retained by our Health Information Management Department. Your medical record is maintained in a strict, confidential manner as required by state and federal laws. We will not release information contained in your medical record without your written authorization or as permitted by law. You can download a Release of Health Care Information form.
You can also request a copy of your medical record by calling the Health Information Management Department at 360-814-8462, between the hours of 8 a.m. and 4:30 p.m., Monday through Friday or email email@example.com.
Medical Information Network - North Sound
A local initiative to benefit our community, Medical Information Network-North Sound (MIN-NS), connects care resources to coordinate and improve care of patients in Skagit County and beyond.
MIN-NS provides secure Health Information Exchange (HIE), secure email, secure video-conferencing, and secure online form tools supporting delivery of medical and social care.
Originating from medical community discussions which began in 2008, MIN-NS was formed in 2009 as a non-profit, community organization supported by Skagit Regional Health, Island Hospital, grants and subscription revenue.
MIN-NS Health Information Exchange reaches across gaps in connectivity between hospitals, clinics and other healthcare services allowing providers at different locations on different electronic and paper systems to securely share patient care information. Records and reports in the HIE can be viewed and filtered according to your needs, then downloaded, printed or securely mailed to others on the patient's healthcare team. Access to the clinical record is supplemented with online forms for exchanging referrals, patient care plans, and advanced directives.
When patients need care outside the North Sound, MIN-NS provides connections to resources in Western Washington and throughout the state and nation with connections to two statewide Health Information Exchanges and secure direct email.
In 2014, MIN-NS leadership in care connectivity was recognized when they were awarded one of 15 federal grants to support development of rural care delivery networks. The grant is being used to support telehealth conferencing and extend the HIE to include the behavioral health provider community.
